Evaluation Process

Consultation & Intake

The process begins with a free 20-minute phone or virtual consultation, so that we can discuss your concerns, reasons for seeking an evaluation, and how Cognitive Canvas Psychology can best meet your needs. During this consultation, any questions you may have will be answered and recommendations will be given which may include a specific type of formal evaluation. All costs will be provided upfront, and a consent form for services will be signed before moving forward.

Assessment Day

Testing sessions will be scheduled and completed in a manner that best meets your availability. We conduct our testing sessions by appointment, 7 days/week, in 2-3 hour blocks. For a comprehensive psycho-educational assessment, typically 2-3 sessions are required but may vary depending on the areas of assessment, how quickly the examinee moves through the items, and the number of breaks taken.

Post-Evaluation

All assessments and any rating scales provided to the parent and/or client will be scored, and this information, along with data from interviews, observations, record reviews, surveys, etc., will be integrated into a comprehensive report. The report should be finalized within 10-15 business days from the completion of assessments, rating scales, and observations (if applicable).

Feedback Session & Recommendations

A feedback session will be scheduled to review all assessment results and the comprehensive report. Copies of the report will be provided, as well as copies of all corresponding bell-curve visuals. Recommendations will be given and any remaining questions will be answered.

Payment Policies

  • Evaluation fees will be provided at the initial consultation.
  • A $200 deposit is collected upon booking the testing session(s).
  • Remaining payment for assessments is requested in 2 installments:
  • 50% of the balance at the start of 1st testing session &
  • 50% at the start of the feedback session, where written report will be provided
  • Payment for counseling services are due at the time of services.
  • Payment for translation services are requested in 2 installments:
  • 50% of the payment upon project agreement
  • 50% upon delivery of translated written report

Accepted forms of payment:

I accept cash, cashier’s checks, personal checks, and direct wire transfers. Credit/debit cards are also accepted via a HIPPA-compliant platform.

Insurance:

At this time, Cognitive Canvas Psychology is considered an “out-of network” provider, and we do not accept insurance. However, we are happy to provide you with an itemized “super bill” which can be given to your insurance company for consideration of reimbursement. We do not, however, guarantee that our services will be reimbursed by any specific insurance provider.

Cancellation Policy:

We kindly ask that if you need to cancel or reschedule a session, that you do so no less than 48 hours prior to a scheduled session.

Location & Alternative Sites

  • Our main office is located at:   1302 Madera Rd. #44, Simi Valley, CA 93065.
  • Main office location boasts ample parking, lobby area, indoor and outdoor lounge/waiting areas for parents or clients taking breaks, complimentary refreshments, clean restrooms, and nearby dining options.
  • For assessments, in instances where our location is too far for you to come to us, arrangements can be made for alternative testing sites. Please ask, so that accommodations can be made.
  • Counseling sessions are available via teletherapy or in-person. A discount is provided for sessions held via teletherapy.
